  • Please consider my request for the following dates to have time-off from work:

  • SHC policy is to receive time-off requests by the 16th of the month prior at the minimum. If you are submitting this late (not by the 16th of the month prior and at least two weeks in advance), please explain why:
  • Please remember that time off must be approved and is not considered fully granted until you have received a notice of approval from the SHC agency office.

    *A confirmation copy of your submission will be emailed to the email address you have provided. as well as to Sinai In-Home Care.
